Job Summary:

 Under the direction of the MD - Geriatrics & Palliative Care, the Nurse Practitioner provides assessment and disease management skills in conjunction with the management of all patients in long-term care centers, skilled care facilities, and homebound services. Assist the Medical Team with those services in which he/she is trained and competent to perform.

 Responsible for the identification, evaluation, and coordination of all health care services provided to the MHC population of patients with high risk, intensive care needs for heart failure management. Interfaces with the OCHP Hospitalist for post-hospitalization planning and case management.  

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.  Other duties may be assigned.

               1.      Works collaboratively with the OCHP Hospitalist team (acute and skilled) Clinical Services team.

2.      Perform daily rounds at assigned skilled facilities. Manages an assigned case load, following established protocols and systems for case management in collaboration with physicians and specialty services.
3.      Develop treatment plan based on medical evaluation, medications and diagnostic studies.
4.      Manage home bound patients as required to facilitate development and execute of a care plan
5.      Manage long-term patients to prevent exacerbation of disease process.
6.      Monitors select high-risk patients through the High Intensity Clinic, in collaboration with OCHP Hospitalist, for post-hospitalization planning and for management of acute episodes of care.
7.      Works in concert with medical management to develop protocols for effective management of chronic illnesses such as congestive
