If you have a Medicare plan:

  1. Call your plan. You can find the phone number on the back of your plan member ID card. 
  2. Ask if the plan includes Monarch in its network. If it doesn’t, say you want switch to Monarch. Note: you can switch networks once a month.
  3. Choose a Monarch doctor.
