If you have a Medicare plan:
- Call your plan. You can find the phone number on the back of your plan member ID card.
- Ask if the plan includes Optum Care Network–Monarch in its network. If it doesn’t, say you want switch to Optum Care Network–Monarch. Note: You can switch networks once a month.
- Choose an Optum Care Network doctor.
If you don’t have a Medicare plan:
- Find a plan that’s right for you.
- Choose an Optum Care Network doctor.
