If you’re in the hospital for a period of time, you may find yourself under the care of a hospitalist. This is a specially trained doctor who works in a hospital. This doctor partners with you, your family and your primary care doctor to keep you as healthy as possible.

When it’s time for you to leave the hospital, you'll need a care plan to keep you on the path to getting better. The hospitalist and your primary care doctor will work together to create that plan.

Sometimes after a hospital stay you may go to a skilled nursing facility (SNF). There, you'll find specially trained doctors called "SNFists." They will also work with your primary care doctor to keep you as healthy as possible.

Both hospitalists and SNFists work closely with your primary care doctor. When it's time to return home, they'll make sure you have the care and resources you need.
